BioSphere Human Optimization ("BioSphere," "we," "our," or "us") partners with BioSphere Physician Partners, PC, an independently owned and operated professional medical corporation, along with its licensed medical providers (collectively, the "Affiliated Providers"), who deliver all clinical and medical services — including evaluations, diagnoses, prescriptions, and ongoing medical care — to clients when appropriate.

These Affiliated Providers maintain full and independent control over all aspects of patient care, including clinical decisions, prescriptions, diagnostic interpretations, and medical oversight. BioSphere does not practice medicine, nor does it interfere with or control the professional judgment of any licensed provider.

By engaging in BioSphere's programs, you understand that:

• Wellness and strategy services are provided directly by BioSphere's certified health and performance professionals, who do not diagnose, treat, or prescribe.
• Medical services, when applicable, are provided solely by the Affiliated Providers through a separate medical entity operating under its own licensure and compliance standards.
• BioSphere facilitates coordination between clients and Affiliated Providers for the purpose of integrated health optimization and communication efficiency.
• Any telehealth or prescription services are governed by the terms, privacy policies, and applicable state laws of those Affiliated Providers.
• BioSphere Human Optimization may assist with scheduling, data integration, and client communication, but all medical advice, prescriptions, and treatment plans are the responsibility of the Affiliated Providers.

Your privacy and the security of your personal health information are important to us.

BioSphere Human Optimization uses Healthie, a secure healthcare platform, to store and manage client records, appointments, communications, forms, and health information. Healthie is designed to support HIPAA-compliant handling of protected health information (PHI) and employs industry-standard security measures, including encryption, access controls, and secure data storage.

Your information is used only for purposes related to your care, wellness strategy, program administration, and authorized communications. Access to your information is limited to authorized BioSphere personnel and healthcare professionals involved in your care.

We do not sell or share your personal health information with third parties for marketing purposes. Information may only be disclosed with your authorization, for care coordination, or as otherwise required by law.

As part of providing personalized strategy and coordinating care with licensed clinicians, BioSphere Human Optimization may collect demographic and health-related information (for example: name, date of birth, gender identity, race/ethnicity, address, emergency contact, insurance details, and limited health history). This information is collected to: (1) deliver and tailor strategy services; (2) facilitate medical consultations and care coordination with licensed clinicians of BioSphere Physician Partners, PC; (3) bill for services and administer memberships; and (4) evaluate and improve our programs.

We treat demographic and health information as protected health information (PHI) when it is associated with your identity and will protect it in accordance with applicable privacy and data-protection laws. Demographic information may be used in de-identified or aggregated form for internal quality improvement, population health analytics, and research; such aggregated data will never identify you individually.
